TRANSCRIPTS:
To order transcripts, see the Guidance Secretary for the
required form. This form must be completed at least TEN
(10) SCHOOL DAYS BEFORE YOUR DEADLINE. This is necessary
because the guidance office is closed during the holidays.
The
guidance secretary will prepare your transcripts. The first
two (2) transcripts are free for seniors. The third and all additional
transcripts requested are $3.00 each. Please pre-pay. You will be notified through your first/second
period teacher when your transcript is ready to be picked
up in the Guidance Office. Transcripts may be picked up
before school, after school, and during lunch. All financial
obligations to the school will have to be cleared before
picking up transcripts. The form above should be filled out and returned to
Mrs. Finewood in the guidance office.
Many
colleges require a copy of your final transcript to
confirm your graduation.

Sometimes a college will
request your first semester grades, especially if you have been deferred and
are still interested in attending the school that deferred you.See the links above for a request form for semester grades to be sent to colleges.
Return the form to the guidance office.
COLLEGE APPLICATIONS:
Request the application from your selected college's admissions office. Myers
Park's Resource Center, in the AG building, may have the applications that
you need, but if we do not have what you need, you must request it from the
college.
Photocopy the application
to use as a rough draft. This will prevent you from making mistakes on your
actual application. Read the entire application before beginning, so you will
be sure to follow instructions and meet deadlines. Type or print using black
ink, and answer all questions. Remember that your application represents you
to the college. Make a copy of the completed application, including all essays,
to keep for your records.
RECOMMENDATIONS: If
recommendations are needed from either your counselor or teachers, ask them
as soon as possible, preferable in the early fall. Failure to plan on
your part does not constitute an emergency on their part.
Have forms turned into
your counselor by:
- 10-15 for early decision/early
action
- 12-1 for regular decision
To help your counselor
write an effective recommendation, please print THIS
FORM and fill out the STUDENT section. Have your parents and a friend
fill out their parts and give 2 teachers an evaluation to fill out. These
should all be returned to your counselor and will remain confidential.
Ask the appropriate teachers
if you may use their names or if they will write recommendations for you.
Provide stamped, addressed envelopes to teachers, if the recommendations are
to be mailed directly to the schools. Give the teachers at least TEN (10)
school days before your deadline.
If you need a written
recommendation from a counselor, see her at least TEN (10) school days before
your deadline. Remember that the counselor may need to talk with you and/or
your teachers. We believe that recommendations should be personal and contain
pertinent information about you. Therefore, you will need to fill out a student
profile form and return it to your counselor before she begins writing your
recommendation. This is in your best interest, but it takes time. Late requests
may result in your recommendation not being completed before the college deadline.
Counselor recommendations will be mailed directly from the school.
TEST SCORES: It
is your responsibility to have your SAT and ACT scores sent directly to the
colleges from the College Board and American College Testing. The code
for MPHS is 340680.
MAILING: You are
responsible for assembling and mailing the completed application packet. This
includes sending your transcript and your test scores.
